Bay Zoltán Nonprofit Ltd. for Applied Research participates in the project Personalized ICT solution to reduce re‐hospitalization rates in heart failure elderly patients suffering from comorbidities (PerHeart).

The goal of the PerHeart project is to develop an info-communication technology that can be used to reduce the rate of repeated hospitalization of patients with heart disease.

In order to achieve this, it supports patients in complying with therapy and lifestyle changes, by providing personalized feedback, revealing risk factors, and analysing patient profiles for professionals and providing data that helps in the interpretation and prediction of diseases.

The PerHeart system will be tested and validated on the basis of data collected in three countries (Poland, Denmark, Italy) in the framework of pilot studies over a period of 14 months, with the involvement of 50 heart disease patients and their caregivers in each country.

Implementation period:  01/09/2020– 31/12/2023
